RESOLUTION

Title: Authorizing Agreement with Care Plus NJ Inc.

 

WHEREAS, the Borough of Hillsdale provides for an Outpatient Counseling Program and related services for Hillsdale adolescents and their families; and

 

WHEREAS, Care Plus NJ with its main offices at 610 Valley Health Plaza, Paramus, NJ provides a quality program of this type; and

 

WHEREAS, payment for this contract period will be the amount of $11,048.00; and

 

WHEREAS, the Chief Financial Officer has certified, in accordance with N.J.S.A. 5:30-14.5 that sufficient legally appropriated funds will be available for this purpose subject to appropriation of funds in the 2008 municipal budget.

 

NOW, THEREFORE, BE IT RESOLVED, that the Mayor and Council of the Borough of Hillsdale authorize Administrator Harold Karns to execute the agreement between the Borough of Hillsdale and Care Plus NJ Inc. for mental health related services from January 1, 2008 to December 31, 2008.
