Certified Copies have the raised seal and are always issued on State of New Jersey safety paper. Certified copies may be used to establish identity and are legal documents.
Customers can order a vital record by mailing the form (SEE BELOW) or by coming in person to the Borough of Hillsdale. We do not accept emails or fax copy requests.
Walk-in hours: 8:00 am - 3:00 pm Mon-Fri. You do not need an appointment to obtain certified copies. For records other than your own or your child's, please see below on how to prove relationship.
To request a copy of a vital record, you must submit:
- A completed application (the form is below)
- A copy of the proof(s) of your identity (See form for list of Valid ID)
- If you request records by mail and your ID does not have current address, you must send copy of secondary ID (see form).
- The correct fee - $25 for One certified copy, $2.00 for each additional certified copy of the same record, requested at the same time. Payment is accepted in the form of a check or money order payable to the Borough of Hillsdale. No cash is accepted.
- Proof of your relationship to the person listed on the vital record (See Proof of Relationship below)
- Marriage certificate if your name has changed due to marriage
How to prove relationship for certified copies of non-genealogical records:
- your own birth certificate: if you have assumed your partner's last name you must also provide a copy of your marriage/civil union certificate to link the name on your current ID to the name on your birth certificate.
- your child’s birth certificate: Valid ID only. You should be listed as the parent on record,
you don’t need any additional documents.

you must provide a copy of your birth certificate.
- your grandparent’s vital record
you must establish that you are the person’s grandchild by providing proof that links the name on your ID to the name of the grandparent.
For example, if you changed your last name after marriage/civil union and want a grandparent’s vital record, you must:- Provide your marriage/civil union certificate to show your name at birth,
- provide your birth certificate to identify your parent, and
- provide the parent’s birth certificate to identify the grandparent.
Amendments to Vital Records:
If you require to make any changes to a Vital Record, you must contact the Registrar by phone or email. 201-497-1552 or 201-497-1562 After the Registrar has all the information necessary to process the request, then an appointment will be scheduled to sign paperwork. There is a $50 fee for all amendments to Vital Records, paid prior to processing any corrections or changes.
Registration of Vital Statistics
The Health Department processes and maintains custody of the following documents related to Vital Statistics for the Borough of Hillsdale only.
- Registration of all Births, Deaths, Marriages, Civil Unions and Domestic Partnerships that occur in Hillsdale.
- Acceptance of Marriage, Civil Union and Domestic Partnerships license applications.
- Issuance of Certified Copies for Vital Records for any Birth, Death, Marriage, Civil Union or Domestic Partnership that has occurred in Hillsdale; following guidelines set forth in Executive Order Number 18.
Vital records are not public records and cannot be searched online, nor can they be requested under OPRA. New Jersey law protects and restricts the release of vital records.
